Journey tag not loading — ads showing loading dots but never filling

Site: inbetweenpictures.com (Squarespace)

What's happening:

Journey script shows zero matches in Network tab — it's not loading at all
All DoubleClick/GAM calls returning 400 Bad Request with CORS errors
Pubmatic returning 502
Mediavine loading dots appear but no ads ever fill
Session RPM down 99.8%, Impressions/Session down 99.8%

What I've already checked:

Code injection is clean
No CSP meta tags
No conflicting scripts
Cookies & Privacy settings reviewed
